Blood Pressure Review

If you have been asked to monitor your blood pressure at home, you can submit your readings using the online form below.

Alternatively, you can print a paper copy of the blood pressure recording form, complete it at home and bring it into the practice for review.

Please ensure you record your readings accurately and include all the requested information.

If you have been advised by the surgery to submit your blood pressure readings on a regular basis, please submit this form.

Smoking status

Measure you blood pressure in the morning and the evening with two
readings each time, 1 minute apart, and record the lower of the 2 readings. Record your blood pressure for at least 4 days or as requested.
